My Care Matters - Enabling holistic care in hospital environments

Project aim:

I helped to improve the mycarematters end-to-end service experience. From awareness, to create a profile of holistic care needs to care providers being able to access that profile easily in the future.

Mycarematters is a social enterprise developing an online platform where people can upload the information they would like healthcare professionals to know about them, in the event they are admitted to hospital and are unable to communicate their needs and preferences.

Approach:

We conducted some user research with people who have loved ones with dementia and with people who had dementia. This helped us to gain a better understanding as to what type of holistic information they would like to be able to easily pass onto potential care providers. This enabled us to co-create holistic information profile templates.

Impact and outcomes:

My care matters holistic profiles are now being used across care homes, hospitals and within families. They have helped to create a further holistic care environment by making personal holistic information easily accessible to care providers.
